left\"\u003e其实，集采的目标和老百姓的希望是一致的，就是让老百姓用得起药。目前，集采已累计节省四千多亿元医保基金，这不是个小数字，它客观上降低了国家的医疗负担。现在，集采已经被写进了政府工作报告，是一定会被执行下去的。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e\u003cstrong\u003e张伟滨：\u003c/strong\u003e有很多。出门诊时，我常遇到专门来开进口药的病人。有时，我会告诉对方：“你要的进口药没进集采，所以给你开的是带量的药。”有些病人会说：“我不要这个，我宁愿自己花钱去买进口药。”有些病人接受是接受了，但多少是有点不高兴，觉得医院没有好药了。\u003c/p\u003e\u003cp left\"\u003e\u003cstrong\u003e张伟滨：\u003c/strong\u003e美国早就在砍药价了。药过了专利保护期后，降价是必然的。为什么我们要设置专利保护期？就是为了让药企在专利保护期内收回成本。过了专利保护期后，就应该放开限制，让任何国家的企业都能去仿制，降低药价，让老百姓都吃得起药，这是在世界各国通行的做法。\u003c/p\u003e\u003cp left\"\u003e我讲一个做法比较极端的国家，印度。在印度，哪怕在专利保护期内的药品，它照样允许“强仿”，质量不差，价格较低，因为印度是世界人口大国，医疗负担很重。其他国家对印度的这个特殊政策没有办法，但印度老百姓是欢迎这个制度的。在电影《\u003c!--SECURE_LINK_BEGIN_1--\u003e我不是药神\u003c!--SECURE_LINK_END_1--\u003e》中，中国病人也会去印度买药。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e\u003cstrong\u003e经济观察报：其实这样的制度不利于一个国家的创新药行业发展。\u003c/strong\u003e\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e\u003cstrong\u003e张伟滨：\u003c/strong\u003e对。中国人口不比印度少，但中国政府没有采取印度这样的政策。我们尊重药品专利保护期，这是尊重知识产权，我们参与到药品创新的世界格局中去。目前，中国创新药并不完全是依靠药企自己赚的钱去研发的，我国政府对企业的创新药研发有很强的支持力度和很大的政策扶持。当一类新药上市后，政府不会马上对这样的创新药启动集采。事实上，目前的集采针对的往往是有很长临床使用时间的基础药、普药和部分仿制药，一些仿制药对应的原研药已经在中国市场卖了很多年了，这种药和创新药是有区别的。\u003c!--MID_AD_1--\u003e\u003c!--EOP_1--\u003e\u003c/p\u003e\u003c!--PARAGRAPH_1--\u003e\u003cp left\"\u003e\u003cstrong\u003e张伟滨：\u003c/strong\u003e我没和他们讨论过，但我看过他们的建议，他们的本意也是希望集采政策能变得更合理，更能满足广大病人的实际需求，更好地监督被集采药企业做好集采药，保证集采药的质量和疗效。我觉得医生存在不同看法是正常的。我们不单单要让老百姓知道集采的好处，也要让医生知道集采药的质量是如何保证的，因为医生是具体用药的主体，是要对患者负责的，如果医生对集采过程不够了解的话，那必然会对政策产生疑虑。\u003c!--MID_AD_2--\u003e\u003c!--EOP_2--\u003e\u003c/p\u003e\u003c!--PARAGRAPH_2--\u003e\u003cp left\"\u003e\u003cstrong\u003e经济观察报：第十一批集采已设置“锚点价”避免极端低价，你认为政策的调整是否能改变部分公众对集采药品“低价低质”的认知？\u003c/strong\u003e\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e\u003cstrong\u003e张伟滨：\u003c/strong\u003e我认为对集采政策的这种调整是非常有必要的。在政策改进之外，我们还必须把这些信息告诉百姓和社会，让大家知道有关部门在通过什么机制去保证“低价优质”。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e有关部门在集采过程中，一定要避免出现过度的恶意竞价现象，要从制药成本上去把控药品价格的合理性。如果有一款药，内行人一看就知道其报价远远低于成本价了，这就是不合理的。低价不能以牺牲成本为代价，因为厂家要生存就必须有利润，否则为了不亏本，企业一定会想办法把成本压到更低，那就有可能造成药品的质量和安全性问题。\u003c!--MID_AD_3--\u003e\u003c!--EOP_3--\u003e\u003c/p\u003e\u003c!--PARAGRAPH_3--\u003e\u003cp left\"\u003e\u003cstrong\u003e张伟滨：\u003c/strong\u003e首先，集采药品都需要通过一致性评价。另外，我认为一定要建立一个监督平台，药监局要不定期对集采药品进行质检和飞行抽查，而且一定要抽查正在市面上流通销售的药品，而不是从厂家拿药品去检测。通过这样的方式，可以持续评价集采药品的疗效和不良反应累积率，并把这些信息上传到政府专门网站上，让老百姓随时能查阅。比如，当病人买到一款集采降压药，他随时可以到相应网站去查，看这个药有没有被药监局稽查过，看它的效果到底有没有保证。这种飞行检查对厂家也是个约束，它们就不敢在过了一次性评价后不继续严格约束自己，就不敢在正式生产环节缺斤少两。\u003c!--MID_AD_4--\u003e\u003c!--EOP_4--\u003e\u003c/p\u003e\u003c!--PARAGRAPH_4--\u003e\u003cp
